Get the Large, They run pretty tight.
Were can i order the long sleeve rashguard ?

Mike Dewitt is also as shredded as they come. his are actually custom made for him. The size is extra schmedium...
Patches are hard to come by these days, I know you can get your hands on a F&V patch. We have some from Chicago North, with the logo in my Signature, I'm sure Josh would could make you a deal for one or two if you asked nice. :)
