First dude, before I did anything I would make sure she has clearance from her doctor. She sounds like she can be a ticking time bomb. She doesn't need to do anything but light cardio for a very long time. Shes not gonna burn that many calories until she gets that heart strengthened up anyway. She needs to be most concerned with calories and diet at this point. The most help you can do with her is to help her put together a good meal plan and coach/motivate her to stick to it.

Remember, Diet is a 4 letter word. It also implies itself to be temporary. Never use "diet" in front of her. "Lifestyle change" "eating clean" is better choices. Don't put her on a calorie regimen at first. Just eliminate the "dirty". She can eat as much chicken breast, salad, fish, veggies etc. that her stomach can fit in it.. This usually fixes most folks main problems..
The biggest and best "diet" change the average person can make is start drinking only water. Once that hydration is cleaned up then you can start cleaning up the eating.. Again... Good luck bro. Lol
