The gag reflex isn't so different from a very common fist choke from mount where you just ram a first into a person's throat from the side and bring the opposite forearm under the neck to support the "choking" arm. I think it's probably the first thing I learned a while back when I started in gi, and the guy held his own sleeve to secure the grip/submission.
It's a dick move but it's still technically a blood choke because you are aiming for the carotid artery. The beauty of the gag reflex is that, in conjunction with a submission like the triangle and a stubborn opponent, it's an additional arsenal you can use to finish the fight.
